The following form is divided into several sections which you may complete in any order by selecting the sectional tabs. Fields marked with a [ * ] are required, but the more information you can supply the stronger your application will be.

If you have not already done so, please ensure that you read the descriptions and requirements for your chosen membership category on our Membership page. If your application is successful, you will be sent an invoice and banking details for payment.

The current annual membership fees are as follows:

Ordinary Member (Reviewer, Lead Practitioner, Practitioner, Environmental Manager): N$600
Apprentice (Emerging Practitioner, Emerging Environmental Manager): N$300
Student (Learner Practitioner): N$120

Section A: Basic Information

Membership Class*

Please ensure that you have selected a membership class that you are qualified for. Entering an inappropriate class may delay your application, or lead to it being rejected.

If you are applying to be either a Reviewer or Lead Practitioner you must also include a completed motivation document. The templates for the motivation documents are available to download from our documents page. These should be included within your ZIP file of supporting documents uploaded in section F.

Section D: Experience and Core Competancies

Core competencies

Provide a list of your core competencies including detailed information on the experience gained and/or work undertaken in order to develop your core competencies. Please focus on environmental assessment related experience, and include project dates and descriptions for each item. The executive committee may request proof of experience at their discretion.

Please add any additional information that you feel may help the executive committee to judge the appropriateness of your professional experience.

Section E: Professional Memberships and Bio.

Professional Memberships

Enter details of your membership of any professional bodies or other relevant organisations (involved with environmental assessment or with regard to broad environmental issues). Please remember to upload proof of your membership status for the organisations you have listed below, on the file upload tab (Section F).

Short Bio.

Enter a short biography of 100 words or less. This will be included on the public website listing if you checked the permission box in section A of the form.

Section F: Document Uploads

Please upload your supporting documentation below. Files should be combined into a single zip file with your full name as entered in section A as the filename.

Please upload all of your supporting documentation here in the form of a single zip file. The files should include your CV, along with certificates for any qualifications and memberships listed in your application. If you are applying to be either a Reviewer or Lead Practitioner, you must also include your completed Motivation letter (see Documents page for blank templates).
